You can begin the adoption process right away by completing an application!
Due to the personal information requested in an adoption application, an online form transmitted via email is not advisable. Therefore, a downloadable form has been provided for your convenience. Simply download the application and print it out. The completed application may then be faxed to us at (925) 827-9396 or mailed to us at this address:
